What is the best way to build a distributed VoIP enterprise system? How many servers would be required and what would be the potential pitfalls?

There are many factors such as WAN link speed, locations, services available, manufacturer and system capabilities, so there is no one set answer for this question.

For instance, some manufacturers can provide load balancing and application level filtering, while some require separate networks for voice communications.

I would suggest that you work with your manufacturer AND your telecommunications providers to get the best solution for your business. In some instances, your telecommunications carrier can provide these services on an outsourced basis for some of your smaller sites.
